Growth Hormone Secretagogue
Tesamorelin is a lab-made copy of a natural body hormone called GHRH (growth hormone-releasing hormone, the hormone that tells the pituitary gland to release growth hormone). It uses the full 44-amino-acid GHRH chain (amino acids are the building blocks of proteins) with a small chemical group added to the front to make it more stable. In studies it is described as a longer-lasting version of GHRH, used to study the growth hormone system and how the body handles belly fat.
Tesamorelin switches on the GHRH receptor (the docking point on cells that GHRH normally acts through) on the growth-hormone-making cells of the pituitary gland, which triggers release of the body's own growth hormone. The added chemical group on the front is studied for protecting it from being broken down by the body's enzymes, so it works longer than natural GHRH [1]. Researchers use it to study how this on-and-off, GHRH-driven release of growth hormone affects a growth signal called IGF-1 and how the body handles fats [1].
Clinical trials (with a placebo comparison and neither participants nor researchers knowing who got what) in adults with HIV who had extra fat build-up around the middle used imaging to measure changes in deep belly fat (the fat packed around the organs). The trials reported drops in this fat in the study groups over the treatment period [1]. Researchers used these studies to look at how switching on the growth hormone system with a GHRH copy affects belly-fat stores compared with placebo [1].
A later clinical trial measured effects on liver fat and on fats in the blood, using imaging to measure how much fat was in the liver alongside the belly fat, in selected groups of patients [2]. This research looks at the links between switching on the growth hormone system, belly fat, and fat stored where it usually should not be [1][2].
