Retatrutide: The Triple-Agonist Peptide and the Research Behind the Hype

Retatrutide (research designation LY3437943) is the peptide that pushed the incretin field to three receptors at once: GIP, GLP-1, and — the new addition — the glucagon receptor. Adding glucagon agonism is counterintuitive at first glance, since glucagon raises blood sugar. The rationale in the literature is that glucagon-receptor engagement can increase energy expenditure and act on hepatic lipid metabolism, while the GLP-1 component offsets the glucose effect.

What the phase 2 data showed

The pivotal report is a phase 2, double-blind, placebo-controlled trial (Jastreboff et al., 2023) in adults with obesity. At 48 weeks, least-squares mean weight change reached −22.8% at the 8 mg dose and −24.2% at 12 mg, versus −2.1% for placebo. Among participants on the higher doses, a large majority crossed the 15%-reduction threshold. These are among the largest reductions reported in a peptide obesity trial to date, which is precisely why retatrutide draws so much attention.

The liver-fat substudy

A prespecified substudy in participants with metabolic dysfunction-associated steatotic liver disease (MASLD) reported striking reductions in liver fat: mean relative decreases exceeding 80% at the 8 and 12 mg doses at 24 weeks, with normal liver-fat content (under 5%) achieved by most participants on those doses. This positioned retatrutide as a compound of interest in liver-metabolism research, not weight alone.
